Dr. Praveena Jeereddi is Board Certified in Internal Medicine. She is originally from Claremont, CA and attended the University of California, San Diego for her BA degree in Psychology. She received her medical education from American University of the Caribbean. She completed her Internal Medicine residency at Catholic Medical Center of Brooklyn/Queens. She has been practicing in this community since 2001 as Hospitalist and currently as a SNFist to improve the continuum of care from the hospital. She has served many roles in the medical group, hospital and health plan.

Dr. Tobin Panicker is Board Certified in Family Medicine. He grew up mostly in sunny southern California, went to UC Berkeley and did his medical school at Ross University. He continued his education at Mercy Family Medicine Residency Program in Janesville, Wisconsin. He was voted most outstanding physician in Residency. He is a proud member of our team since 2013. He continues to participate in mission trips throughout the world and loves to serve our community.
